The following application is for the disclosure of adoption related information under section 48.1 or 48.2 of the Vital Statistics Act. Please read the following before starting the application process, as it discusses the eligibility requirements, application process, what documentation you are entitled to, and other important information that may impact your results.

As the adopted person, you must be at least 18 years old to apply for Post Adoption Birth Information (PABI).

 

As the birth/other parent named on the adopted person’s original birth registration, you must wait until the adopted person is at least 19 years old before you can apply for PABI.

As the adopted person, if you are eligible for Post Adoption Birth Information (PABI) and the Office of the Registrar General has received a No Contact Notice from a birth parent, you will be required to sign a written agreement stating you will not contact or attempt to contact that birth parent. Once you have signed and returned the agreement, a copy of any statement completed by that birth parent, containing any medical or family history, or reasons for not wanting to be contacted, will be given to you along with your uncertified copy of your original birth registration and/or registered adoption order.

 

Under the Vital Statistics Act, you are prohibited from directly or indirectly contacting or attempting to contact the birth parent who has filed a No Contact Notice. If you are convicted of violating a No Contact Notice, you may be subject to a fine of up to $50,000.

 

As the birth/other parent, if you are eligible for PABI and the Office of the Registrar General has received a No Contact Notice from the adopted person, you will be required to sign a written agreement stating you will not contact or attempt to contact the adopted person. Once you have signed and returned the agreement, a copy of any statement completed by the adopted person, containing any medical or family history, or reasons for not wanting to be contacted, will be given to you along with uncertified copies of the adopted person’s birth registration(s) and registered adoption order, if applicable.
