Application for a Plant Milk Grader CertificatePursuant to the Milk Act (R.S.O. 1990, c. M12) and Regulation 761
You will be asked to:
⢠Attach a letter from the Dairy Farmers of Ontario indicating the allotted volume of milk, the milk classes, and under what program that milk supply is being issued.
⢠List all products to be manufactured at this plant, indicating the species of milk, the classes and subclasses of cow milk, if applicable and any specialty product(s) (e.g. organic, grass fed, DHA, Ultra Kosher) Refer to Regulations 761 and 753.
⢠Provide the names of the Plant Milk Graders employed at the facility.

Personal information on this form is collected as it is necessary for the proper administration of section 88 of R.R.O. 1990, Regulation 761: Milk and Milk Products under the Milk Act, R.S.O. 1990, c. M.12. It will be used for the purpose of issuing a certificate to act as a Plant Milk Grader, including contacting the applicant.
